2001 Volkswagen Eurovan Cooling Fan Assembly

Has anybody had any problems with their cooling fan? We were going down the road and the engine started to shake... we go out and open the hood and a chunk was out of one of the cooling fans.... we were able to get home. But now need to replace the broken fan. Does anybody know where to get instructions for replacing the fan? Or the best place to order the fan? On the fan is written 700-959-455k Siemens SN6 made in Canada F6-241-035... another number on there is 129023
