Those actuators are usually configured with linkage rods. Looks like there is a hole in that lever. I bet you are missing a rod that is supposed to go from the actuator to the lever. If you can figure out where the actuator was mounted, it should be obvious. For reference, I have attached a pic of how the actuator is used in the newer beds. Note that the triangular thing is a pivoting lever to increase the throw:
