Phoenix Cruiser 2100 - slide failure

Hi - has anyone here had a catastrophic failure with a Phoenix slide? I bought a 2021 2100 6 months ago ... and fast forward, the slide fell out (completely disconnected) and almost crushed my husband - we didn't even use it yet?

Dealership claims no responsibility. Phoenix tells me they want to 'fix it' and send it back to me. I don't want it back.

My understanding is that this is a catastrophic unprecedented failure
