there are plans which offer coverage nationwide- they cost more- much more and the in network providers in your area may not offer these- this is key,
what is offered in your State/County may be different from those in the next county or State- as with auto insurance in the US it is state by state
I live in PHX, my insurance was only good in AZ and I travel for a living..
when I was working a show in Las Vegas- the plan was to try to drive to nearest in-network facility in Kingman AZ...or at least make it 30 miles to the AZ border and call 911 for an ambulance.

for 2017 we will go without.
only one provider offers for 2017 in my county- and we have previous very poor experience with them
there will be a penalty we'll have to pay, but it is a fraction of the premiums for a provider we already know is useless.
so a crap shoot
