This info relates to the photos in my previous post. Parts, cutting, drilling and welding cost about $200. Not beautiful, but functional....remember that some of the factory racks like this have counter springs to help lift the wheel, this relies on a fairly heavy lift.
I bought a receiver tilt mechanism from Amazon, the part which tilts will hold a 2 x 2 steel bar, this bar was from my parts "bin"
I wanted the spare to be located above the bumper and tucked in close to the door, closer than the old door mounted mount, so I inserted the bar into the tilt mechanism, and measured the distance to the lug nut holes in the wheel and had the bar drilled 5/8 to mount the tire, in this case two holes.
I needed a receiver extender so the 2 x 2 bar would be the correct distance from the back door, in this case I needed to shorten and drill new mount holes in the tilt mechanism so it could be secured closer to the back door.
I also added a second receiver to the hitch so that I could continue to use a bike rack mounted in the center, the bar on the hitch which this is mounted to is on an angle, so it offered me about an inch to adjust the receiver fore and aft to help adjust the the distance the tire is from the back door.
I added this tilt mechanism

This is the extra receiver I used which is mounted to the existing hitch

This is the receiver extender, there are a bunch out there, but this is one of the shorter ones
